Brain death and organ donation: Observations and interventions

    Abstract

    Organs transplanted from brain-dead donors perform worse than those transplanted from living donors. This means that the state of brain death negatively effects the performance of future donor-organs. This thesis describes, by using experimental rats, negative effects of brain death on the one hand, and tries to prevent these effects on the other hand by treating brain-dead rats. One of the negative effects described in this thesis involves the speed at which brain death is induced. We showed that the speed of brain death induction influences the extent of organ damage in brain-dead rats. This could be useful information as to ascertain how to treat brain-dead donors and deciding whether to discard certain organs. Another negative effect of brain death we observed was decreased anti-oxidative capacity in the kidney resulting in oxidative stress. This decreased capacity was more pronounced after slow brain death induction. Furthermore, we showed that decreased renal anti-oxidative capacity could be related to the fact that intrarenal perfusion decreases during brain death. To counteract renal oxidative stress, we administered brain-dead rats the anti-oxidant MnTMPyP. This led to decreased oxidative stress in the kidney and resulted in improved kidney function. Treating brain-dead donors with this compound could therefore lead to improved transplantation outcomes.
