Lifestyle and work-related factors in hand eczema: a cross-sectional questionnaire based study in the Dutch general population

Background: Data on chronic hand eczema (HE) and severity of HE in the general population is limited. Furthermore, results regarding lifestyle factors are conflicting and studies on non-occupational wet exposure, occupations not considered as high-risk, and socioeconomic factors are scarce.

Objectives: To investigate the prevalence and severity of (chronic) HE in the Dutch general population and to report the association with lifestyle, (non-)occupational exposure, and work-related factors.

Methods: In 2020 a questionnaire, including questions on HE and exposure, was sent to 135 950 participants of the Lifelines Cohort Study. Data on lifestyle, work-related, and socioeconomic factors were collected from baseline (2006-2013). Binary logistic regression models were performed.

Results: In total, 58 198 participants responded (42.8%).The 1-year prevalence of chronic HE was 4.7% and the lifetime prevalence of moderate-to-very severe HE was 6.4%. Smoking and obesity were associated with HE, chronic HE, and more severe HE. Chronic stress was associated with HE and chronic HE, and less physical activity was associated with more severe HE. Both occupational and non-occupational wet exposure were positively associated with HE. Furthermore, positive associations for high-risk occupations, occupations not considered as high-risk, and higher levels of education were found.

Conclusion: Chronic and severe HE represents a considerable proportion of HE patients. Preventive strategies for HE should focus on avoidance of all exposure to wet activities, regardless of origin. In addition, job tasks instead of job title should be taken into account. Lifestyle interventions might be considered in the treatment of HE to improve overall health.
